MONTHLY COMMISSIONS INFORMATION - As an Independent Distributor please provide your bank account details to enable your local Nu Skin office to deposit commission payments in your account. If you need to make changes, please call Distributor Support at 1-800-487-1000, option 6.

I authorize Nu Skin Enterprises United States, Inc. (“Nu Skin”) to deposit any commissions/bonuses I have earned to my account at the financial institution designated by me and I represent and warrant that I have the authority to do so. This authorization replaces any previous authorization and will remain in full force and effect until Nu Skin has (i) received written notice from me of my withdrawal from the direct deposit program, and (ii) a reasonable opportunity to make such a change pursuant to my notice.
Only the Applicant or Spouse/Co-habitant of the Distributor Agreement, or the Primary Participant of a Business Entity that is applying to become a Distributor can revoke this authorization or revise any direct deposit information submitted with the authorization.
I agree that I must notify Nu Skin immediately (i) prior to changing or closing the above account, or (ii) if my financial institution changes my routing number or account number. I acknowledge and agree that it is my responsibility to make sure that the commissions/bonuses have been deposited in my account before transferring funds from the account. Nu Skin’s limits on liability are set forth in Sections (B)(7) and (F)(4) of the Distributor Agreement.
