Replacement Request Form

Fill in this form in order to apply for a replacement. Please note your eligibility will be evaluated based on the below information. You will need your order number as well as the damaged device to complete it.

 

Nu Skin guarantees your device to be free from defects in materials and workmanship for a period of two years from the original date of delivery to you (more info on page 9 of Quick Start Guide). This warranty does not cover damage to the product as a result of misuse or accident.  You may be required to present a formal sales receipt showing the delivery date.
