The “Heart-to-Heart Charity Walk 2015” will be held on November 1 (Sunday), 2015 at the Peak. We now sincerely invite you to participate in together. Since 2005, NU SKIN Hong Kong has been taking the initiative to support the Children’s Heart Foundation (CHF), helping the children with heart disease to complete the surgery successfully. In a bid to continue to display our “Force for Good” and help more people in need, NU SKIN will fully support the “Heart-to-Heart Charity Walk 2015” organized by the CHF. We have supported the charity walk for the past ten years, raising nearly HK$5,000,000 in total and become a “Diamond Sponsor” of the event. Besides, we were also awarded the “Royal Cup” (the greatest amount of funds raised) by the organizer for the past nine consecutive years. It has proven that NU SKIN business partners are all so generous in supporting charity works with one heart and one goal.

Route:Morning Trail at the Peak (about one hour)

Transport:By shuttle bus provided by the organizer or other means arranged by participants themselves
Time of Waiting for Shuttle Bus:8:00am to 8:30am (late-comers will not be served)

Enrollment:Please fill in and submit the registration form, together with donations ( crossed cheques be made payable to “Children Heart’s Foundation” or bank pay-in slips), to NU SKIN Plaza (10/F., Lee Gardens Two, 28 Yun Ping Road, Causeway Bay) or Executive Partners before October 23, 2015 (Friday). NU SKIN willpass your donations to Children’s Heart Foundation (CHF) on your behalf.
*For business partners in Mainland China and Taiwan, please mail the filled registration forms with cheques(Only accept cheques in HKD) to 10/F., Lee Gardens Two, 28 Yun Ping Road, Causeway Bay [Attention: Force For Good]. NU SKIN will pass your cheques to Children’s Heart Foundation (CHF) on your behalf. “Force for Good” T-shirts can be redeemed at NU SKIN booth on the spot of the event day.
T-shirt Arrangement:Each participant will receive one “Force for Good” T-shirt. When you submit your registration forms and donations, our staff will give you the T-shirts according to the number of participants written on the form. Please wear the T-shirt on that day

Note:
- Please submit the registration forms together with donations (crossed cheques or bank pay-in slips), otherwise the application will not be accepted.
- Please fill in the total number of participants, number of people taking the shuttle bus and mailing address for official receipts on the registration forms.
- Each participant should donate HK$200 or above*.
- In response to protect the environment, if you already have a “Force for Good” T-shirt, you may choose not to receive it.
- Once you have received the “Force for Good” T-shirts, please sign on the registration form. If the sizes that you want have run out, our staff will arrange other sizes for you.
- Limited paid parking spaces are available at the Peak Galleria.

*For example, you want to join the event with other three business partners. You need to submit only one registration form and fill in the total number of participants and the names of these 3 business partners respectively. The total amount of donation on the registration form should be HK$800.
