Release of Information
Our success at Nu Skin Enterprises is due, in large part, to our worldwide reputation for honesty and integrity in all of our business dealings. Our Code of Business Conduct can be summarized by the simple phrase "do the right thing." Background checks have become a necessity under new regulations imposed by both the Sarbanes-Oxley legislation, and by credit card associations in a concerted effort to protect people from identity theft and fraud.
In order to maintain that standard and adhere to the new regulations for public companies, Nu Skin Enterprises, Inc. has found it necessary and in the best interest of the Company, our customers, and employees to acquire background checks on all employees and prospective employees.
I understand as a result of these concerns and to more fully protect all concerned, that background inquiries may be made on me including consumer credit, criminal convictions, motor vehicle, and other financial reports. These reports will include information as to my work habits, performance, education and experience along with reasons for termination of employment from previous employers. Further, I understand that the Company will be requesting this information from various Federal, State, and other agencies.
I authorize without reservation, any party or agency contacted to furnish the above mentioned information and release all parties involved from any liability and responsibility for doing so. I hereby consent to obtaining the above information from any licensed agents. This authorization and consent shall be valid in original, Fax or copy form. I further authorize on going procurement of the above mentioned reports at any time during my employment (or contract) and acknowledge receipt of the Consumer Reports Notification below regarding pulling of such consumer reports.
Consumer Report Notification
I am hereby notified that a consumer report may be obtained from a consumer reporting agency, other agency or directly by the Company for the purpose of evaluating myself for contract work, employment, promotion, reassignment or retention as an employee.
The report may contain information bearing on my personal credit worthiness, credit standing, credit capacity, character, general reputation, personal characteristics or mode of living from public or private record sources or through personal interviews with associates, former employers, and educational institutions
Applicant's Certification and Agreement (Read carefully before signing)
All information provided by me is true and correct to the best of my knowledge. I understand omissions or misrepresentations may be cause for rejection or, if employed, may be just cause for subsequent dismissal. I hereby authorize any former employer, person, firm, or corporation listed hereon, including this Company, to answer any questions and agree to hold all persons harmless for giving any and all truthful information within their knowledge or records. I understand this is a preliminary application and not a contract to employ me. Furthermore, in the event I am employed, I understand that employment at this Company is "at will," which means that either I or the Company can terminate the employment relationship at any time, with or without prior notice, and for any or no reason not prohibited by law. All employment is continued on that basis. I understand that my at-will employment status may not be changed or modified (1) by any oral statement to the contrary, whether made during the application process or during employment, (2) by any practice or procedure in the Company or in the industry, or (3) by any policy manual or other document issued by the Company except by a written employment contract executed by the President of the Company and myself that pertains solely to my employment and that specifically revokes the employment-at-will relationship. If employed, I agree to comply with all Company policies and procedures as a condition of continuing employment.
